Open a new image 300 w x 300 h. Activate the Path Drawing Tool
,
create a rounded rectangle 285w x 138h.
Click on the edit path object, toggle, delete the nodes on the left and right sides, then click on each line and change to curve.
Right click on your color square and select gradient fill. Click and change the first square to #E8E8E8 and then click on the second color square changing to #A4A4A4. For the "fill type" select the third one (arrow pointing downward right) and click OK.
Add a border width of 6 using black for the color.

Activate the Path Drawing Tool
,
create a white circle 22 diameter.
Activate the Outline Drawing Tool and create a black circle 28 diameter.
Place circles as shown in image to your left.

Click on your metal tag so the ants are marching then right click and convert object from text/path to image.
Activate the Magic Wand Tool
,
click on the plus sign on the attribute tool bar. (The ants are still marching around the tag, keep them marching!)
Click on the white circle with your magic wand, right click expand/shrink by 3 using shape circle and keeping original soft edge.
Press the "Delete" key on your keyboard.
In the layer manager you will see your tag now has a circle hole! While your there, right click and delete the white circle you created as you no longer need it.
The image to the left with the yellow background is just to show you how your metal tag will look.

Press the space bar on your keyboard to deselect the magic wand.
Activate the Magic Wand Tool again, this time click on the black border of your metal tag.
Right click expand/shrink by 1 using shape circle and keeping original soft edge.
Go to Edit, Trace Edges, Selection Marquee and select the default settings.
On the attribute tool bar, change horizontal deform to 3D object and the color to #D4D4D4.

Lastly, activate the outline drawing tool and click on the black circle outline you previously made.
O the attribute tool bar, change the color to #D4D4D4 and the mode to 3D object.
Activate the path drawing tool, right click on the color square, select "gradient fill". Change the first color to #A50000 and the second color to #B93100.
